1. What personal information is collected from you

When you visit the Site we collect various personal information which may include your name, address, contact details, IP address, and information regarding what pages are accessed and when. When you register as a member of LINk:MK we will collect details such as your name, date of birth, contact details GP Surgery disability and ethnicity. The registration form (online or offline) states what these details are used for and you have the option to not specify some fields. If you supply such information we are legally bound by the Data Protection Act 1998.

2. How your information is used

We may use this information in the following ways :

  • Send you regular communication, via email or post, as part of your membership
  • Send you communications that you have requested or that may be of interest to you
  • Monitor and evaluate our communication strategy
  • Monitor our membership with regards to its representation of the local community
  • Inform our annual report to the Department of Health

We are committed to protecting the personal data of our members and participants. Any details you give us will be held in accordance with the Data Protection Act 1998. LINk:MK is the sole owner of the information collected on our site or via registration.
